The commentator said we'd stuck with a flat back 4 when Cresswell came off. If he had changed the formation, then totally fair enough.

I’d say it was a 3 even when Cresswell was on. He just had license to overlap with Soucek or Rice then dropping back.
Fornals was basically left wingback and Emerson took his role when he came on.
It was quite fluid, but the heatmap shows a clear 3.

It was a back 3 (or 5 when defending) from the start.
Fornals and Coufal playing as wingbacks.
Cresswell, Zouma and Kehrer as CB's.
Ogbonna replaced Cresswell at LCB.
Emerson replaced Fornals LWB.
You could see all 5 in a line throughout the game when Tottenham had the ball.
At no point did we play 4 at the back.
